Occupational Therapist (Centralized Eligibility Team) (Caregiver Support)

The incumbent serves as a core member of a VISN-level interprofessional group of clinical reviewers-the Centralized Eligibility Team (CET). The CET Occupational Therapistposition requires advanced knowledge and understanding of not only the services and benefits available for caregivers from both federal, state and private sector resources but the statute, regulations, and policy that govern the program. The Incumbent will review the Veteran's medical record summary, most recent History & Physical, findings of recent testing at the facility, caregiver assessment and care team input. The team member will present their and other OTs respective findings to the interprofessional team. The team will meet and render a multi-disciplinary decision regarding clinical eligibility, based on all eligibility requirements. If deemed eligible, the team will also determine the appropriate level of caregiving required (tier level). Occupational Therapist (OT) at this level practice independently and are responsible for comprehensive assessment of functional and occupational roles using standardized and non-standardized evaluation tools. OT's select and provide direct occupational therapy interventions and procedures with varying degrees of complexity. Duties include but are not limited to:
Works with interdisciplinary team members at VISN and facility levels to ensure coordinated and directed eligibility determinations for the PCAFC. Coordinates with other Regional CETs and VACO CSP to develop and implement strategies for PCAFC eligibility, tier level, discharge, and appeals process and performance improvement. Conducts comprehensive clinical review of patient records to identify the presence of Veteran need for assistance with personal care services and/or supervision or protection needs daily. Maintains understanding of eligibility associated with service connection and conducts review of eligibility documents, as needed, to determine service connected condition meets eligibility requirement. Analyzes data obtained in record review for use in determining clinical eligibility for PCAFC. Documents information utilized in clinical eligibility determination and eligibility assessment tools. Conducts Peer Reviews to ensure national consistency in eligibility determinations. Collaborates with and contributes knowledge to a community of practice across VISN's and Regions with CSC's and CET members Utilizes knowledge of PCAFC to coordinate with interdisciplinary regional clinical eligibility team members to evaluate appeals received by other VISN's as they relate to clinical eligibility. Assists the VISN with MISSION Act implementation, as it relates to CSP and PCAFC expansion. Receives, reviews, and dispositions PCAFC initial eligibility decisions; reassessments; and appeal decisions for clinical eligibly determinations. Executes position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care. Provides professional oversight and consultation for occupational therapist involved in caregiver programs, monitors and evaluates clinically appropriate. Work Schedule:

(a) Bachelor's degree in Occupational Therapy AND two (2) years of progressively independent experience as an occupational therapist; OR (b) Bachelor's degree in Occupational Therapy AND two (2) full years of graduate education in a related field; OR (c) Master's Degree or higher in Occupational Therapy. (2) Individuals must be a graduate of a degree program in occupational therapy approved by the Accreditation Council for Occupational Therapy Education (ACOTE) or predecessor organizations. This is inclusive of an internship (supervised fieldwork experience required by the educational institution). ACOTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA). Candidates must possess a current NBCOT certification as an OT. State Licensure:
Candidates must possess a full, current, and unrestricted state license, to practice occupational therapy in a State, Territory or Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia. Loss of Credential. An employee in this occupation who fails to maintain the required state or territorial licensure and the NBCOT certification, both current and in good standing, must be removed from the occupation, which may also result in termination of employment. GS-12 Occupational Therapist (Full Performance Level) In addition to the basic requirements stated above, the following qualification criteria must be met:
(1) Experience. Completion of one year of progressively complex experience equivalent to the GS-11 grade level directly related to the position being filled. (2) Demonstrated Knowledge, Skills and Abilities (KSAs). In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of occupational therapy principles and techniques consistent with current clinical standards based on OT theory and evidence based practice. Knowledge is inclusive of physical, occupational, cognitive, and psycho-social functional deficits. b) Ability to collaborate and communicate orally and in writing with all internal and external stakeholders. (c) Ability to use critical analysis, clinical reasoning, and creativity to independently solve complex problems related to adapting and modifying assessments, treatment plans, activities and procedures to meet the needs of patients. (d) Skill in procuring, fabricating, adjusting, adapting, and modifying orthoses, splints, and adaptive equipment for activities of daily living (inclusive of durable medical equipment). This position requires potentially long periods of continued walking, standing, and sitting. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. In the performance of duties, the incumbent may be required to drive and/or ride in GSA-vehicles.
