Registered Nurse, Substance Use Disorder Initiative, PRN

JOB SUMMARYThe primary purpose of this position is to provide case management, medication monitoring, self-management training/patient education, and supportive counseling as needed to patients with substance use disorders.
This role involves assessing patients and family members for appropriateness for participating in the program or providing referrals to other agencies.
Also, in this role, the RN will provide ongoing follow-up and support for all patients with substance use disorders.
This position is PRN.
SPECIFIC RESPONSIBILITIES Participate in daily huddles with patient care team(s) Collaborates with behavioral health providers, peer support specialists, providers, clinical staff, and other community services Participates in team meetings Assess the physical, functional, social, psychological, financial, and environmental needs of patients Identify a cost-effective, comprehensive plan to meet the patient and family needs Enrolls new patients into programs and services as appropriate Develops individual plans to include case management, medication monitoring, self-management training, patient education, and limited counseling and provides ongoing monitoring and reassessment of needs Participates in the ordering and administration of specialty SUD injections Facilitates referrals to outside agencies as needed Facilitates group meetings as needed Grant-funded project support Participate in the ongoing design and implementation of nurse-led protocols and co-visits Provide panel management support to identify quality care gaps and coordinate outreach Identify and perform pre-visit planning for a variety of visit types Review and triage medication refills requests Assist with the management of urgent and emergent patient care situations Performs phlebotomy, clia-waived lab tests, processes specimens, and starts IVs as needed Participate in annual competency training of clinical staff Float between clinics as needed to meet the needs of the department Participate in nursing phone triage Serve as a preceptor for new clinical staff Provide care management support such as transitions of care calls and hospital follow-ups within the clinic Participates in process improvement of new and existing workflows Actively embraces and supports divisional and organizational initiatives This role description is a general description of the essential job functions.

SPECIFIED SKILLS COMPUTER Excellent skills in Microsoft Office including Word, Excel, PowerPoint, and database applications required.
FOREIGN LANGUAGE Spanish speaking skills preferred.

Qualifications:
Bachelor Degree in Nursing.
PREFERRED
Qualifications:
Two (2) years relevant experience or comparable combined experience and/or education.
One (1) to two (2) years of nursing experience in an ambulatory setting.
Telephone triage experience.
REQUIRED LICENSES Current unencumbered North Carolina nursing license Current CPR certification SCHEDULERegular attendance on-site is an essential function of this position.
Typical business hours are Monday - Friday, 8:
00 am to 5:
00 pm (or flexed to best meet the needs of the clients and/or the Division); 40 hours per workweek; weekend, holiday, or evening coverage is occasionally required.
Work hours will need to be flexible in order to respond to special work assignments, or evening activities, as requested by the team leader.
