00 Salary/yearDescriptionPURPOSE OF THIS POSITION:
The primary purpose of this position is to provide support and clinical supervision to all team members regarding implementation of an individual's Plan of Care.
ROLE AND RESPONSIBILITIESoEnsure NC licensure standards and Medicaid guidelines are followed.
oDevelop plans of care for individuals supported.
oMonitor all services that the team provides to ensure that they are being executed in accordance to the plan of care.
oSupervise all direct care team members in the implementation of supports.
oAct as a liaison between Liberty Corner and all outside agencies involved in provision of or arrangement for services of individuals supported.
oEnsure that the records of all individuals supported are maintained properly according to NC licensure standards.
oAssist Lead Direct Support Professionals in creating and maintaining a budget for each site.
oProvide monthly supervision to all members of the care team including but not limited to corrective action, continuing training, one-on-one meetings, and annual paraprofessional.
oEnsure that all time worked by team members is documented correctly and turned in to the finance department in a timely manner.
OTHER POSITION REQUIREMENTSoEnsure the promotion and protection of individual human rights; ensure confidentiality of any information related to persons receiving residential supports; ensure that individuals keep and maintain personal possessions; ensure that persons are free from abuse, neglect, exploitation, or punishment.
oProvide encouragement to individuals to achieve specific skills or behaviors in independent living, socialization, meal preparation, budgeting, money management, and recreation and leisure activities.
oMonitor the overall health and safety of the individuals served including providing information to medical professionals (psychiatrists, general practitioners, specialists, etc.
).
Assuring that issues and concerns about health and safety are brought to the attention of team members when they arise; and that direct care staff are communicated with regarding treatments and interventions.
oFacilitate staff meetings.
Attend meetings and staff training as requested.
oCommunicate effectively and follow instructions, both oral and written.
oMaintain program standards as required by regulatory agencies.
oDemonstrate effective interactions, training techniques, and interventions for direct care staff.
oTurn in billing and all other required paperwork on time.
oDemonstrate and model effective interactions, training techniques, and interventions.
oOther duties as assigned by the Operations Director, Human Resources Director or Executive Director.
oMonitor the daily activities of staff and ensure they are completing responsibilities efficiently.
oConsult with the HR department regarding staff performance and issue written and/or verbal warnings as necessary.
Review consultations with the HR department to ensure all disciplinary action is conducted in accordance with state and federal regulations.
oAccuracy is essential in all aspects of this work.
Accuracy and timeliness in record keeping is critical to the agency adhering to state, licensure, and rules to avoid penalties and paybacks.
QUALIFICATIONS AND EDUCATION REQUIREMENTSThe required minimum qualifications for this position include:
oBachelor's degree in a human service fieldo2 years full time, post-bachelor's degree accumulated MH/IDD/SU experience with the population.
oValid NC driver's license, acceptable driving record and appropriate automobile insuranceoSatisfactory background check and Healthcare Personnel Registry reportoProficient and confident with applicable software, hardware, and technologyoCompetency in the following areas:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
